First post/log, so I'm going to try to give all the info requested. Please let me know what else I'm missing and I'll edit/post. Thanks!


Relevant History: 35 year old male diagnosed a year ago with Klinefelter’s Syndrome (XXY) and prescribed TRT. (Klinefelter’s is a rare genetic mutation that essentially presents as primary hypogonadism)

Prior to TRT my natural total test was around 250 ng/dL. Currently I’m prescribed 160 mg of test cyp a week and I float around 850 ng/dL total test and 45 pg/mL estradiol. I inject 2x a week. Since starting TRT a year ago, I’ve lost about 55 pounds and 11% BF. Currently, I weigh 198 pounds and am sitting at 23% BF at 5’10 inches tall (BF% is according to a renpho digital scale). I’ve been lifting 3-4x a week for about 6 months and haven’t been able to put much muscle mass on, despite eating 160+ grams of protein a day (filling the rest in with carbs/fat while still running a deficit). Just because of the way my clinic does TRT, I’ve been able to accumulate enough extra test to run a 24 week cycle and I won’t have labs due again until after that, so I have time to bring my levels back down prior. I’ll admit that diet is my weakness and I’m still learning my way around the gym. I skip meals a lot and fill a lot of my protein in with protein shakes and bars and protein-added snacks (Premier Protein drinks with 30g protein, barebell bars and those puff bars with like 15-20g of protein in them, and stuff like Greek yogurt and beef jerky). I do get at least one solid meal in the evenings though. I lift in the morning, mostly doing the whole-body circuit at PF, with some other machines added in.



Current Medicines: Mounjaro 15mg/weekly (I’m a type 2 diabetic, comes with Klinefelters); Jardiance 25mg daily; Atenolol 50mg 2x daily (anxiety and bp); Minocycline for acne



Plan:

Start eating 200g of protein daily, filling in remaining calories with healthy foods, tracked with MyFitnessPal. Continue a mild caloric deficit.

Begin lifting 5 days a week and hire a lifting coach that I can see in-person.

Donate blood every 60 days at ARC.

Week 8 full blood work. Week 16 full blood work.

Week 33 post cycle full blood work (for clinic).

I’ll have an AI on hand, but I’m trying really hard not to use it.



Run the following test cyp cycle (daily injections):

Week 1 – 300 mg

Week 2 – 400 mg

Weeks 3-27 – 500 mg

Week 28 – 400 mg

Week 29 – 300 mg

Weeks 30-33 – 160 mg

Week 33 Clinic Blood Work

Week 34 – 250 or 300 mg cruise



Goals: Lose 10% more BF while gaining and retaining as much muscle as possible.

If there’s anything I could/should add on to help, I’m all ears. Or any other advice anyone could offer for accomplishing my goals. If I’m just truly wasting my time and a couple vials of cyp, I’ll accept that, so I guess tell me if that’s the case, too.

As far as specific questions I have the following: is 3 weeks at a lower dose enough for my blood work to come back acceptable for the clinic? Anyone use any meal-kit style services that are high protein that you recommend? Is 200g of protein daily enough, or should I aim higher?

Only issue I see here is the high dose of test for 30weeks. Not super healthy.
Also you don't need to titrate up. Start at 500mg if that's what you want to run. It takes 5-6 weeks to reach full blood saturation.
Also if at week 33 you have clinic bloods I would stop the test and go back to your TRT dose at week 25-26 to give you time to get your bloodwork back in check before you see the doc. You may also want to get your own bloods prior so there are no surprises at the clinic.
 
sgentz is right. your hemotcrit will be elevated on even a slighly higher test dose.

not good at all. you will open yourself up to blood clots and high bp from thick blood

its hard to say because we dont know your full dose and injection schedule yet.

Good start to the log. You have discovered the secret......diet. Regardless of hormones and training, if the diet isn't locked in, you won't see the results you are looking for.
 
Bros. titrating or pyramiding gear is pointless. the gear already has esters that do it for you.
 
Learn about steroid Esters and how they do the work for you.

This idea that you have to move the dose up and down, I'm not really sure where you got this idea from
 
It's good. That you donate blood. That's smart.

But you can donate blood all day when you're overdosing gear like this, for a long. Of time. It will have health ramifications.
 
I'd be curious to see where your numbers are with your blood work.
Make sure you follow your doctor's advice on the trt.

Seems like your weight's been all over the place. Even with your genetic issue and your syndrome that you have, you can still get in the best shape your life and we will help you.
 
What kind of protocol are you on when it comes to your thyroid?

I know you are dealing with type 2 diabetes, but are you on anything for that?

Keep being consistent, and don't let any type of Health issues get in your way of being your best.
