- Training Experience (in years):
- Briefly describe your current workout routine, including frequency/reps, types of exercises, and weight for each exercise.
- Daily Caloric Intake:
- Macronutrient Split: Protein/Carbohydrates/Fats (%)-If not known, list goal daily protein intake for now.
Meal Plan: try to list 2-3 different meal options for each- Breakfast:
- Snack:
- Lunch:
- Snack:
- Dinner:
- List any supplements, dosage, and frequency.
- Recent Blood Work: Yes/No
- If yes, please provide relevant levels (e.g., testosterone, liver enzymes,CBC,E2,CMP,Etc.)
- Do you plan to do Pre, Mid and Post blood test during your cycle
- Cycle Goals:
- Compounds Planned for Use:
- Dosage and Frequency:
