Human chorionic gonadotropin (hCG) is a hormone produced by trophoblast tissue, which is found in early embryos and eventually becomes part of the placenta. It plays a crucial role in pregnancy and has various medical applications.

HCG is a glycoprotein composed of two subunits:
  • Alpha subunit: Identical to other glycoprotein hormones like LH, FSH, and TSH
  • Beta subunit: Unique to hCG, providing its specific biological activity
The primary functions of hCG include:
  1. Maintaining the corpus luteum during early pregnancy
  2. Promoting progesterone production
  3. Supporting fetal growth and development
  4. Modulating maternal immune response to prevent rejection of the fetus
The discovery and understanding of hCG have evolved over time:
  • 1920s: Selmar Aschheim and Bernhard Zondek first described hCG and its presence in pregnant women's urine.
  • 1930s: Dr. Georgeanna Jones discovered that hCG was produced by the placenta, not the pituitary gland as previously thought.
  • 1960s: The development of immunoassays allowed for more accurate hCG detection without using animals.
  • 1972: Judith Vaitukaitis and Glenn Braunstein at the National Institutes of Health developed a sensitive hCG assay that could distinguish between hCG and LH, making early pregnancy detection possible.
  • 1978: The first home pregnancy test based on hCG detection became available to consumers.
HCG has several important clinical applications:Pregnancy detection and monitoring:
  • Qualitative tests: Detect the presence of hCG in urine or blood
  • Quantitative tests: Measure exact hCG levels, useful for monitoring early pregnancy progress
Diagnosis and management of certain conditions:
  • Gestational trophoblastic diseases
  • Testicular and ovarian germ cell tumors
Fertility treatments:
  • Triggering ovulation in assisted reproductive technologies
  • Supporting luteal phase in some fertility protocols
Male hypogonadism:
  • Stimulating testosterone production in men with certain types of hypogonadism
HCG levels can be measured in both blood and urine (Pregnancy tests). Blood tests are more sensitive and can detect pregnancy earlier than urine tests. In early pregnancy, hCG levels typically double every 48-72 hours.Abnormal hCG levels or patterns may indicate:
  • Ectopic pregnancy
  • Miscarriage
  • Multiple gestation
  • Gestational trophoblastic disease
  • Some female cancers can Elevate hCG levels (False positive Pregnancy Test)



HCG (Human Chorionic Gonadotropin) plays a significant role in testosterone replacement therapy (TRT) and can be used both as a standalone treatment and in conjunction with TRT. Here's an overview of its benefits and uses:

Standalone Treatment:​

HCG monotherapy can be used to boost testosterone levels in some men, particularly those with borderline low testosterone. Benefits of HCG monotherapy:
  • Stimulates natural testosterone production
  • Promotes sperm production
  • May improve fertility
  • Potentially fewer side effects compared to TRT
A small study showed that HCG monotherapy increased testosterone levels by up to 263 ng/dL (42.8%) in men with hypogonadism symptoms but testosterone levels above 300 ng/dL. However, HCG alone may not be as effective as TRT in alleviating symptoms for many patients with more severe testosterone deficiencies.

HCG in Combination with TRT​

HCG is often used alongside TRT to mitigate some of the side effects associated with exogenous testosterone use.

Benefits of combining HCG with TRT:
  1. Preserves testicular function: HCG helps maintain testicular size and function by mimicking luteinizing hormone (LH).
  2. Maintains fertility: HCG can help preserve spermatogenesis in men undergoing TRT by maintaining intratesticular testosterone levels.
  3. Improves libido and sexual function: Some men report enhanced libido and penile sensitivity when using HCG with TRT.
  4. Potential cognitive benefits: HCG may contribute to improved cognitive function when used alongside TRT.
  5. Holistic well-being: The TRT-HCG combination often results in improved mood, energy, and overall vitality
