Hi all,

Excited to be here!

Health History

Hoping to start a new chapter here. Have been dealing with low testosterone symptoms since I was about 18 years old (first time ever getting labs). Testosterone levels were in the 300s at 18 and now at 23 years old I am in the high 200s. For the last 5 years I have had a pathologically low libido and have dealt with lots of fatigue and lethargy. I have worked out consistently then inconsistenlty the last 4 years, with the main problem for having to stop in the gym back when I was in undergraduate was that the fatigue in the gym would make it impossible to study in the way I needed (Biology/Neuroscience, pre-med).

I have run the gambit doing everything in my power to avoid TRT. I do have bipolar II, Major depressive disorder, and generalized anxiety disorder, so this included everything from stopping all psychiatric medication to selectively adding some medication back, etc with no luck. I have tried lithium, villazadone, wellbutrin, prozac, lexapro, and currently on Lamictal and trintellix. Further I have used enclomiphene and HCG both with more side effects than symptom relief (none). The last time I ran enlcomiphene it was pharma grade from a clinic at 12.5mg daily for two months. My testosterone level at the end was still only low 500s. So now I am here! I have frozen 6 vials of sperm and my pre-bloods show that my health is in order. I also wore a CGM last month and good there. BP is 120/80.

If there is one symptom I am looking for meaningfully improvement in it would be libido.

Supplements/medications
1. Lamictal 100mg QHS
2. Trintellix 10mg QHS
3. Adderall 5mg IR prn
4. Propranolol 20mg PRN
5. Trazadone 50mg QHS
6. Cialis 5mg QD
7. Fish oil 2000mg EPA 1000mg DHA QHS
8. Magnesium Glycinate 400mg (elemental) QHS
9. NAC 200mg QD
10. Vitamin D + K2 5000iu QAM
11. Multivitamin (LifeExtensions One a Day) QHS
12. Trazadone 50mg QHS
13. Psyllium Husk Fiber (10g split into 2 doses, Kroger-brand sugar-free)
14. Creatine 5g Nutricost

The "Cycle"
After trying alternative methods and living with low testosterone for essentially my entire adult life to this point, I have decided to make the lifelong commitment to TRT. After contemplating and receiving feedbackin my previous thread, I have decided it is best to dial my training back in as well as lock in my TRT dosing before doing anything supraphysiologic. I will be starting 120mg test cyp/week (40mg thrice a week, at the instruction of my prescriber).

Training

My stats now are 23 years-old/6'1", 167lbs/15-17% BF

My training is hypertrophy-focused, I don't care much for being strong anymore. I might get back into benching again. My all-time max was when I was 18 I got 315x3, but now could only likely do 225x1. I enjoy higher rep-ranges for smaller muscle groups in the 10-15 range and fo larger muscle groups prefer the 8-12 range. Legs have always been a struggle due to FAI/hip dysplasia/hip surgery + right knee medial mensicus tear that recently had an exacerbation with a likely posterior horn meniscal irritation / compression flare.

My training plan is as follows, but is subject to change based on studying needs while I prepare for med school admissions exam. I do my best to upload a daily log. Since I am in week 3 back into the gym I am not fixating on numbers, just mainly getting back my mind muscle connection and making sure I avoid injury/strain. Keeping it short and simple

Day 1: Chest/back - chest focused
Day 2: Arms/shoulders- biceps focused
Day 3: Legs- Quad focused
Day 4: rest
Day 5: Chest/back- back focused
Day 6: arms/shoulders- triceps focused
Day 7: legs- hammy focused.
Day 8: rest

For example, my planned chest/back day tomorrow will be:
Machine Bench 4 x 8-12
Lat Pulldown 4 x 8-12
Pec flys 4 x 10-15
Seated row machine 4 x 6-18

Nutrition/Hydration

Diet is currently 1400 calories with 140g protein/97carbs/54 fat. This was to lose weight before cycle to have a good starting point. Did this for the last month and lost a bit over 10 pounds.

Now that I am back into the gym, I'm going to bump this up to 2100 calories daily (may move this number based on results). I study all day and train at night so I partition all my carbs to my workout window. Tomorrow is my first day on this new calorie allotment and diet will be as follows:

Meal 1:
28g Mixed Nuts
Fairlife Mealplan Shake
Macros: 35P/8C/15F - 290 calories
Meal 2:
1 banana
1 scoop whey
1 scoop psyllium husk (5g fiber)
Macros: 25P/29C/2F - 210 calories
Meal 3 (pre-workout)
6oz cooked chicken breast
250g cooked white rice
1 tbsp BBQ sauce
1 tbsp olive oil
Macros: 42P/86C/19F
Meal 4 (Post-workout)
6oz cooked chicken breast
450g raw potato with skin
1 tbsp olive oil
1 tbsp reduced sugar ketchup
2oz blackberries
Macros: 42P/103C/19F - 760 calories
Before Bed
greek yogurt/low-fat string cheese/etc to fit macros
Daily totals
160P/232C/58F - 2100ish calories

I drink 1 gallon of water a day. My potassium is good as long as I have potato and banana. Always aware of electrolyte balance.

Sleep

I sleep 8 hours a night. Track using apple watch. I take magnesium glycinate as well, but 50mg trazadone with 2mg melatoninis what does the trick for me.

Current Physique

Not siked about the way I look but I have some muscle memory so know I will be able to recomp quickly. First photo was taken (relaxed, not flexing) this week.

Lifestyle/cardio
Right now I am incredibly sedentary as I study for medical school admission exam. Usually 10 hours at a computer. I make sure I do 20-30 minutes of zone 3 cardio 5 days a week, and I go for walks as frequently as possible, though I should be more dilligent about getting my steps in and taking more mini-walks throughout my study day.

All labs posted!

Okay I think this is at least a good start to my log! Mods let me know if I'm missing anything!
Amazing first log post! It's in mod que for approval. Let's do this!

I had posted this question in your first thread (non-log thread): If you don't mind sharing (I share all, just see my log for example) what is your diagnosis to need #1,2, and 7 above? I have a full disclosure of my Dx and all Rx meds in my log and I applaud you for sharing those with us. I am currently titrating off both seroquil QHS and valium QHS myself. I never found that trazadone did much for me even at 100mg.

This will be the best place to discuss your Rx meds and Dx's as well if you want others to chime in who can relate. I know I can.

Absolutely brother! I’ve been formally diagnosed with bipolar type II, major depressive disorder, and generalized anxiety disorder.
Currently I’m taking Lamictal and trintellix. These are both 10/10 fantastic medications for me. Trintellix is very expensive, but since I work in the medical field I am able to get it for free!

Okay I think this is at least a good start to my log! Mods let me know if I'm missing anything!
@NerdGuy not sure why you had to start a new log. Not sure what the deal is there but glad you got this log started. Definitely going to be a good one to follow.

Why are you coming off your medications?
Good question.

My Rx meds were all for my heart (unchanged until I consult with cardiologist after my most recent CT scan w/ contrast) and for my diagnosis of Operator Syndrome (see my log for Rx meds and reasons for each). I've been in remission from O/S long enough that all of my meds for this are at the old dose that was required since 2019 and are therefore all too high and I was too sedated/slow during the day (if you see the meds I was on you'll understand). My personality hasn't changed as I lower them and instead my energy levels have gone up. I've reduced the Zoloft (Sertaline) by 25%, reduced Valium qhs by 75%, reduced zopiclone qhs by 100% (I'm done with that) and reduced Seroquil (Quetiapine) by 100%, and Prazosin I haven't reduced yet. My personality is the same including mood except I'm not as drowsy all the way to dinner as I used to be and can actually wake up in the night and be much more 'with it' if need be. My goal is to be able to hit the gym in the morning or any time of the day I want as well as titrate down to the least effective dose that is required (if any are still required).

It may also matter that I've made it no secret I've become very anti-Big Pharma and have always believe that many practicioners (especially GP Family Doctors) are nothing but pill pushers who don't half of what I know about these drugs anyway. If they put half the effort into monitoring my meds after the fact of getting me stabilized then they ought to know that leaving me hanging on these doses forever is horrible for my health in general.

In a nutshell my Rx med monitoring fell by the wayside and they put zero effort into monitoring my required doses (all maxed out) after I was stabilized with them in 2019 vs. what I ought to be dosing at now (after 7 years of therapy and rehabilitation).

The biggest reason of all that got the idea in my mind in the first place was here on EVO. Realizing that diet, training, supplements, better sleep and most of all having a community and brotherhood like EVO helped me build the confidence to realize there's more to healing and getting mentally and spiritually fit than what a pill can offer. So I'm not taking even 1mg of anything extra that isn't necessary moving forward and this is all being supervised by 3 different docs so I'm well monitored and taken care of.

I also have given biweekly voluntary urine samples to prove my sobriety since my sobriety date on 06Jul2021.

Thank you for sharing all that! Firstly, I am so glad you are doing better. That cocktail is certainly strongly sedative and I can imagine you are feeling a lot more energetic since coming off of it. Very happy to hear that you are taking these huge steps on your own while still staying supervised by a doctor. And congratulations on your sobriety! So huge.

I definitely understand why you have become anti big-pharma and have trouble trusting physicians. There are certainly a lot of bad physicians, and this is coming from someone who is going to be a doctor. Psychiatry is an especially difficult field. In fact, back in undergraduate I wrote a 100-page thesis about the history of public sentiment concerning psychiatry entitled "Hope, Disillusionment, and the Psychiatric iIagination: A Historical Framework" where I kind of explored the historical and current failings of healthcare- so I get it.

Lowest effective dose is always best! For some people that means nothing and for some people it means a lot, and both are okay👍. If you ever wanna talk meds or anything feel free to hit me up!

Bro I think I love you! That thesis is right up my alley. This excites me and talking organic chemistry including peptides and medications is my new jive. I left my M.Sc Math/Physics to go into policing and now full circle I'm way more into anything organic chemistry, the brain, and most recently mitochondrial function.

I'd DM you with some questions and we can get the Nerd shit going hard! LOL :p:cool:

Was instructed to start a new thread so here we are! Below I will paste some information from my original post, supplemented with extra relevant info!

Okay I think this is at least a good start to my log! Mods let me know if I'm missing anything!
Thanks for making such a detailed post. I honestly had no clue the other day when I first saw you post that you were only 23 years old. That means you have your whole lifting career in front of you so make sure you do it the right way the next couple years. That will determine how successful you can be as you age. @NerdGuy

Yeah my supplements aren't cheap. It's about $50/month for all of them so not too bad though! Cheaper than HGH lol
$50 a month is a great value.

Was instructed to start a new thread so here we are! Below I will paste some information from my original post, supplemented with extra relevant info!

@NerdGuy very nice start to the log and welcome to evo. You covered all of your bases here and I like how you have planned for the future. Nice work man.

Update 03/09/2026

Has now been 10 days since beginnning TRT and getting back into the gym. No effects from TRT yet as it's still building up, but have seen massive growth in the last week physique wise. Energy still pretty low, especially after workout. Sleep has been 7-8 hours each night consistently. Finally figured out my favorite pinning technique and have gotten no PIP or soreness since

Nutrition
Have locked in my four main meals to be involving chicken, potatoes, rice, protein shakes, and bananas, with psyllium husk, mixed nuts, and blackberries sprinkled throughout. Order of the meals changes depending on if I do a AM or PM workouts, as I like to partition some good carbs to pre and post-workout windows. Example day of eating below, from yesterday.

Supplements
Same as before, but I have added p5p at 100mg eod due to high prolactin on last blood test and some nipple puffiness. I am also worried about my acne coming back as I start tesosterone so I ordered some accutane that I may start microdosing at 10mg ed- still undecided here, will check with my doc first.
1. Lamictal 100mg
2. Trintellix 10mg
5. Trazadone 50mg
6. Cialis 5mg
7. Fish oil 2000mg EPA 1000mg
8. Magnesium Glycinate 400mg (elemental)

13. Psyllium Husk Fiber (10g split into 2 doses, Kroger-brand sugar-free)
14. Creatine 5g Nutricost
15. P5P 100mg eod

Training
Lifts have been really good. This morning I hit legs (posterior chain dominant day) and felt pretty good, those numbers inserted below. My hamstrings are super tight so RDLs were brutal. Back extensions aren't my thing and the glute machine isn't great so next week I will substitute these for hip thrusts. I have decided I'm going to really focus on legs for the first time in my lifting career.

Arms are starting to gain some size back, but still very small compared to how I have looked before. Chest is super weak, but pumps have been good.

Workout looks pretty good mate. If you're going to focus on legs then it might be good to include some quads in the ham day and vice-versa. Otherwise you're only hitting some of those muscles once per week or so compared to twice a week for upper.

Awesome log bud. Great physique to start your log from bro.
Yeah my supplements aren't cheap. It's about $50/month for all of them so not too bad though! Cheaper than HGH lol
I see your taking Kroger sugar free psyllium husk. Check the ingredients to see if it says how much actual husk per serving. Metamucil original, the orange flavour one for example has 3.6g husk per 12g serving., less than 30% weight. Kroger is US right? I took a quick peek, looks like 60% husk, 40% fillers(see attachment, not sure how accurate it is). You should order organic husk for next time, more husk for your buck.

This one is what they call their "premium" psyllium fiber so it has 5 grams per serving but I agree! I've actually decided to add a smoothie daily so that I can get my berries, oats, flax, and psyllium all in one place. Raw psyllium is hard for me to choke down.

I will probably add that down the line! Since I haven't worked out in so long I'm still getting brutal DOMS so for a couple more weeks I'm just giving myself more time to recover

Agreed, its an acquired taste. lol you know whats worse? downing powdered milk thistle in water. 🤣 shit was brutal. i add my ground flax to oatmeal with a scoop of whey. Makes for a quick breaky.

raw psyllium just mix with a big cup of water like 16oz im guessing you're not mixing in enough water @NerdGuy

Congrats on starting TRT brother! You'll notice a shift in energy from the test itself around 4 weeks in, though the massive change comes as a result of accumulated habits, routines, health, and energy that you develop over time while being on test. All coming from the affect of Test making effort feel good.

Quick update 03/12/2026.

12 days into TRT. So far so good. Started isotretinoin at 10mg/day. Diet has stayed consistent, but have gone ahead to add a smoothie because fresh fruit is too expensive. Good place to add some oats, fruit, flaxseed for some more fiber. Gonna see how well I can maintain my lipid profile while adding the isotretinoin.

Acne isn't bad, just something I have always been self-conscious about with how it makes me look a bit younger than my age (its just face acne). I have medical school interviews coming up in a few months and just wanna be confident about my apperance. I won't go above 10mg, though and will be staying on top of my lipids.

Update 03/13/2026

Training

Quad dominant leg day today. Left hamstrings alone as I still have DOMS from posterior chain day 4 days ago. I was hoping to get some more quad stimulus today and had planned to return toleg extension after calf press, but neuromuscular fatigue had me shot- with my quads shaking the whole time I was doing calves. With this being my fourth leg workout in years, can't complain though. Excited to be able to grow from here. Quad pump was 10/10 even though they are quite little.

Nutrition

I was craving a sandwich post workout and was out of pre-workout bananas but kept it pretty tame. Think I will heed the advice of a couple people on here and push my calories to 2400-2700.

I like how you threw in some lateral extensions and machine chest press. @NerdGuy Those pressing exercises are very good. They help develop the chest.

Definitely nice on the quadriceps training. But make sure you balance it out as well and hit your hamstrings. That's good though that you're craving food after your workout means your body wants to grow. @NerdGuy

Definitely looking a lot bigger in the mirror and increasing cuts/vascularity even with the weight increase.

Quick Update 03/23/2026

Currently 23 days into TRT. Not feeling a ton different yet, but definitely noticing better pumps in the gym and maybe a little bit more energy/motivation-especially in the AM. Have gained a lot of muscle with now 4 weeks of consistency in the gym. Have increased my calories to about 2700 now with the muscle gain I've been attaining. Vascularity has also been steadily improving even with increased weight gain.

Still getting pretty severe soreness after leg days. Excited to start trying to increase volume there.

you're training too much, cut sets by 2 so only 2 sets per exercise and add more reps like 15-20 @NerdGuy
