Blood Work TRT bloodwork - Low Ferritin high hematocrit

Hey fellas, I’ve been creeping on the forum for about a month reading logs, gaining some insight on my questions before I decided to post anything. Was an intermittent follower of OLM for years, now I see the underground forum world has evolved and I can’t believe the amount of support this group at evo provides.

Anyways, I plan on starting here with my bloodwork and dexa feedback first, I do plan on starting up a new cycle log with training schedule etc… can do now but this specific issue below has been on my mind before I start a new recomp cycle… already ordered gear, waiting on TD. Exact cycle still tbd, picked up a range of things ti give me options so not quite there with starting until I feel like this bloodwork isn’t an issue.

41, 220, 5’ 9”

-Currently on TRT @ 160mg/week single injection. I’ve been on this protocol, without coming off for about 6 years.
-Currently on 2.5 Trizeptide/week single injection

I have a history of 5-6 cycles from 22 yo to 35 yo. No cycle in past 5 years

My biggest issue currently is I’m fatigued as fuck all the time, I’ve been training hard, admittedly maybe a little over training lately but the fatigue has been around for years now.

I go to trt doc and my gp separately and tell them I can’t fucking breathe or barely catch my breathe working out, yes I work hard but my bodies working harder than it needs to imo. TRT doc looks at my hema and their protocol has been to have me dump, dump dump. So I have been doing tons of blood dumps for the past 3 years just thinking that’s how it is. I started thinking about running another cycle about a month ago and came across this forum where I started researching the issue I’m having.

Talk to my gp about not be able to catch my breathe and he’s got me talking to ENT’s and doing sleep study and all this shit that’s never helped.

After reading tons of info from y’all’s threads I got my bloods done, (results are still trickling in) I’ll post full bloods when complete but in the meantime as I figured based on reading y’all’s wisdom to others, my hemocratic and hemoglobin is high and my Ferratin is bottomed out. DISCLAIMLER THOUGH, I didn’t read enough earlier enough because I did do a tough cardio workout right before this last the blood draw. If I need to retest this based on better timing I can.

As I read more it seems like a viscous cycle with trt of thick blood = blood dump = iron depletion = lethargic feeling but if I add iron then the red blood cells build up again and more blood dumps.

I’m trying to figure out if the low Ferratin is driving my fatigue, again I’m not a smart dude but what I’m learning at this early phase is that the iron delivers o2 to blood and if I’m low I’ll feel like shit.

I feel like I could probably do better with my description but I wanted to get the post in so I could at least start.

TRT doc looks at my hema and their protocol has been to have me dump, dump dump. So I have been doing tons of blood dumps for the past 3 years just thinking that’s how it is. I started thinking about running another cycle about a month ago and came across this forum where I started researching the issue I’m having.

Talk to my gp about not be able to catch my breathe and he’s got me talking to ENT’s and doing sleep study and all this shit that’s never helped.

Your labs point right at the problem, ferritin 15 is low while hematocrit 50.6 and hemoglobin 17.2 are high, so the constant blood donations likely created the exact trap you described where TRT pushes RBC up, then giving blood pushes iron down and leaves you tired, winded, and flat.

I would not start a recomp cycle yet, I would retest hydrated with no cardio before bloods, stop random bloods, and get full iron panel, ferritin, CBC, B12, folate, BP, sleep apnea results, and e2 posted.
at 220lbs at 5 ft 9 with 17% body fat you have a solid base, but get a TRT log up now with training, food, cardio, sleep, TRT, tirzepatide, BP, and all bloods because the EVO family can help you dial this in properly before you make it worse. 15min of your time to start.

Why is a Log Journal important?

  • Doing a log will actually help you way more than just dropping random updates. When you lay it all out week by week you start to see patterns in your body that you won’t catch otherwise.
  • Other guys here run logs all the time and there’s no privacy issue, nobody is asking for your name or anything personal, it is just training, diet, and how the compound feels.
  • The feedback you get is also way better because people can follow along and spot things you might miss. It also helps the next guy who is thinking about running the same compound so you are giving back while you learn.
  • Even if you think you might not stick with it, just starting the log makes it easier to come back and add quick notes. That way you get more out of what you are running instead of keeping it all in your head.

To really guide you we need more info from you:
  • Please share your diet, training, cardio, sleep, supplements etc log with us in a NEW thread, full log journal so our guidance is based on facts you provide.
  • It will take 15-20 minutes max.
  • We have 100s of years of experience between us, so you need to post a LOG Journal with your stats (weight,height,age, years training, cycle history) diet, training, cardio, supplement, sleep details. If you don't log what you eat or train now, open NOTES on phone and start recording it there and paste here. Very easy.

160mgs a week of trt is going to cause thicker blood. that isn't abnormal

this is why i always recommend 100mgs of TRT. i know with myself even if i use just 125mgs of test a week on a cycle i get higher hematocrit

Thanks brother I’ll work on the updated bloodwork, shooting to have it complete by end of week.

Looking at existing threads now for example on log template.

Yeah, the trt clinic never once mentioned a lower dosage over 6 years. They’re prescribing 200mg/week but I’ve been lowering it on my own.

Because your TRT clinic doesn't care about your long-term health. They just want to make you happy in the short term and get more customers. Plus we live in a society of excess. If they gave you 90 mg a week, a lot of people would stop going to them.
When their customer is dead from a stroke, it's very difficult to prove that the clinic was responsible but that's what we're seeing more and more from people.

Smaller test shots EOD blunt the hematocrit climb so you can ease off the donations and let ferritin recover, and iron supps land better on a non donation day.

Not the donation itself. Just saying that instead of chasing a good hematocrit number by donating, I would suggest pinning EOD or more often than he described (he mentioned single injection weekly) because pinning EOD or even E3D will keep the hematocrit steadier in the first place. The blood work does not lie, so it's good that you're not putting your head in the sand. If more people got blood work done, they would also have concerns. You're not the only one.
 
I can’t put in words how it feels thinking this might be something I can actually get figured out. 1 month ago I didn’t know what evolutionary was, heard a podcast then Found the forum. 2 weeks ago I didn’t know what ferritin was… read existing posts started researching… 2 hours ago I posted and I can’t believe this information was unavailable anywhere without having the right guidance. I’m gonna start logging and take advantage of this wisdom. Read your blog post @bss, amazing info. Not smart enough to understand a lot of it but the more I learn about low ferritin the more it hits home to exactly what I’m feeling. I haven’t had the urge to eat dirt but otherwise everything else checks the box. I can’t imagine how many guys are on trt going through the same thing and not getting this kind of feedback.

There’s a lot of things I question 22 years ago old me about doing. I had a few buddies that were starting to juice, I used to compete in Jr Strngman competitions so strength always amazed me. One day my boy handed me a vial, I thing it was EQ or test, Waited months before I tried it but it was short lived. Basically that first interaction was an experiment I’d say my next attempt was 27/28 but that one I was a little more educated with Ang at that time I was travelling around the country on business constantly and was looking for an edge to stay sharp, first real cycle was Sust and Winstol with PCT
 
Headed to gym for workout 1 today (yeah I don’t think yall will be happy with my current routine once I get it posted but we’re going to Destin on the 13th and I’m putting in work) I’m assuming posting the log is best done at the end of the day so I’ll get through the day and post everything in a new log this evening.

Current Situation:
160mg TRT/Week pinned once a week (TRT Clinic)
T3/T4 10.5MCG/40MCG ?!? Idk.... I take what they tell me (TRT Clinic)
2.5mg Tirzepitide pinned once a week (GP)
I first posted in the Bloodwork section of the forum because I've got some issues with high Hemacratic and low Ferritin. I'm starting this log to get to the root of low Ferritin for now because I feel like dog shit. Currently working out twice a day for a few reasons, a) I either work from home, which I can't fucking stand or I'm travelling to different projects throught the Western US in a high stress industry so letting someone else push me through workouts helps me for now b) I believe this low iron thing is causing me serious fatigue so tbh I workout with different trainers one in the morning and one in the afternoon right now because if I don't I feel like I'd never get off my ass to get my workouts in, even though I'm tired all the time I don't sleep well at all. Yes I'm capable of doing it on my own but man I need someone else to bring the mojo and I just hit autopilot to get through my workouts for the time being. Fatigue has been around for a while but I push through by showing up and not thinking about it. It's not me and once I found EVO, just a few weeks ago, I finally saw some hope for regaining my energy. Already getting feedback on some steps to take to level out the bloods before I start any cycle...

PED Experience:
5 or 6 cycles from 20's - 35.... I don't really remember the composition of all of them over the years but once I found Tren I was a believer and I'd say the last 4 probably had Tren

This had me pretty close to where I was wanting to be, dream cycle for me at the time.... I was eating whatever I wanted and stayed strong, thick, hard and not ripped ripped was just the right amount of fuckable chubby muscle... anyways

Planned Cycle to consist of:
I'll be waiting to see changes from the blood issues before starting anything but I did already place an order, before finding EVO and am waiting for TD. I ordered a few different things with the idea that I'd figure out what I want to run when the time comes

Yeah, I'm open to suggestions here. I've never been 100% or even 85% on diet... when I really need to lean out historically I've just jumped into carnivore for 6-8 weeks. I've improved with my eating with the GLP1's but I've lived on chicken wings and queso in the past... Honestly, If I can eat consistently throughout the day the right things then I can do the exact same meal every day for at least a month, it's just that if I forget I'm eating clean then one chip leads to 3 bags.... I make up for lack of talent and education by outworking everyone in the business world I operate in and I've inadvertently followed that same mentality in my personal life, including my fitness but with my fitness I realize the mindset may have worked in my 20's/early 30's but not now. Yes being on cycles has improved my mental states in the past and I do want to get back to that but this time using the right tools and just blasting through.

I had bicep tendocis and a torn labrum on my left side from around 2021 through 2024 before it got so bad by arm was numb... Got the survery to fix both and healed up well. Doc said I have arthritis in my shoulders and to lay off heavy lifts but I just can't... I do want to do heavy lifting smart though.

I developed a bakers cyst on my left knee in mid-2024.. They recommended not draining it and god damn if it didn't take about a year for the swelling to go down and be able to get back into full workouts.. I laid off a lot of legs during that span. I did ambilical stem cells in the knee and I do maintenance PRP shots in my knee and ankle.

It's just me and my wife, no kids and I do have a lot of freedom in life. I've spent a lot of free time doing useless shit and coasting through the whole health side of my life by over working to compensate for bad living. I want to see what potential I can have with my fitness. Last year my wife and I started supporting the local Special Olympics Powerlifting program and honestly they give me the inspiration to want to take control of this whole lack of energy thing I've not been able to solve so I pulled my head out of the sand and am hoping I can be that inspiration for others but I gotta start with me.

I first posted in the Bloodwork section of the forum because I've got some issues with high Hemacratic and low Ferritin. I'm starting this log to get to the root of low Ferritin for now because I feel like dog shit. Currently working out twice a day for a few reasons, a) I either work from home, which I can't fucking stand or I'm travelling to different projects throught the Western US in a high stress industry so letting someone else push me through workouts helps me for now b) I believe this low iron thing is causing me serious fatigue so tbh I workout with different trainers one in the morning and one in the afternoon right now because if I don't I feel like I'd never get off my ass to get my workouts in, even though I'm tired all the time I don't sleep well at all. Yes I'm capable of doing it on my own but man I need someone else to bring the mojo and I just hit autopilot to get through my workouts for the time being. Fatigue has been around for a while but I push through by showing up and not thinking about it. Just saying that instead of chasing a good hematocrit number by donating, I would suggest pinning EOD or more often than he described (he mentioned single injection weekly) because pinning EOD or even E3D will keep the hematocrit steadier in the first place. i was super low ferritin too after my last cycle. Was also suffering from extreme midday tiredness to the point of nearly falling asleep at my desk each day around 1pm. I've been supplementing with Iron Biglycinate to try bring it back up to a normal range and will have full bloods taken again in about a month to check progress.
Good luck with your bloods. After getting some insight on this forum, I'm gonna kick the TRT clinic to the curb, they've never once paid attention to me asking why I'm so tired, they try selling me B Vitamin shots and shit. Anyways I've got an appt with my GP tomorrow, he's told me before that he'll take over my trt and it'll go from $225/month from the clinic to $54/10ml 200mg Test E vial when prescribed through him and he said his first recommendation would be to decrease the dosage. Will know more tomorrow

Anyways I've got an appt with my GP tomorrow, he's told me before that he'll take over my trt and it'll go from $225/month from the clinic to $54/10ml 200mg Test E vial when prescribed through him and he said his first recommendation would be to decrease the dosage. Will know more tomorrow
I switched to Advanced Health and Fitness as my clinic recently. I got a follow up from the "main" dr 2 days after my consult and script as he wanted to go over my bloods more thoroughly and discuss my iron in particular. although i already had a management plan in place to hopefully address it, i was very pleased that they felt it necessary to follow up and offer advice and options on what i could do to raise and investigate the issue further.

Additionally, they agreed to just send me escripts for my primoteston depot so there is absolutely ZERO markup on my scripts. Saved me $180 over my previousy provider (primal zone).
