Hello Evo Family,

Some of you already know I’ve been dealing with a shoulder/rotator cuff issue for a while. I’ve put it off long enough, so I’m finally going to handle it and get the surgery done. The plan is to get past the constant pain/discomfort, recover the right way, and stop worrying about making it worse—so I can (hopefully) get back in the gym pain-free and actually enjoy training again.

I’m starting this log to post updates along the way (pre-op, surgery, rehab, milestones, wins/struggles), mainly to keep myself accountable and also in case it helps anyone else going through something similar.

Diagnosis

Nontraumatic complete rupture of rotator cuff of right shoulder

TECHNIQUE Multiplanar multisequence MRI imaging through the shoulder.

FINDINGS ROTATOR CUFF: Mild supraspinatus and infraspinatus tendinosis. There is a 7 mm full-thickness tear of the supraspinatus tendon at the anterior footprint with a adjacent 3 cm intermediate-grade partial-thickness articular sided tear supraspinatus tendon and anterior fibers of the infraspinatus tendon. There is a partial-thickness articular sided tear with thinning and attenuation distal subscapularis tendon.

MUSCLES: Grade 1 fatty atrophy supraspinatus and infraspinatus muscles.

BICEPS: Mild tendinosis proximal long head biceps tendon with medial dislocation out of the bicipital groove, mild fluid within the tendon sheath.

SUBACROMIAL / SUBDELTOID BURSA: Mild bursitis.

ACROMION / AC JOINT: Mild AC joint osteoarthritis with bulky inferior spurring. Type 2 acromion.

GLENOHUMERAL JOINT / LABRUM: No labral tear. The articular cartilage and capsular structures are normal.

BONE MARROW: No osseous Bankart or Hill Sachs lesion.

IMPRESSION

1. Mild tendinosis of the rotator cuff complex. There is a 7 mm full-thickness tear at the anterior footprint of the supraspinatus tendon with an adjacent 3 cm intermediate-grade partial-thickness articular sided tear.

2. Partial-thickness articular sided tear distal subscapularis tendon.

3. Mild tendinosis proximal long head biceps tendon with medial dislocation out of the bicipital groove associated with the subscapularis tendon tear.

4. Mild subacromial subdeltoid bursitis.

5. Mild osteoarthritis of the AC joint with bulky inferior spurring.

6. No discrete labral tear noted.

Nice first post. Detailed and picturesque too. We will help you through this bro especially the mental aspect.

Day Before Surgery – Pre-Op Update

Had my final appointment with the surgeon today to get a clearer picture of what I’m walking into tomorrow and what the recovery is going to look like.

Here’s what he told me:

  • Around 3 months in, I should be back in the gym doing light lifting.
  • Around 6 months, I should be back to lifting at full capacity (weight-wise).
  • But… he also said there are certain movements I’ll likely need to avoid going forward — things like lateral raises, incline bench, military press/shoulder press, etc. And closer to that 3-month point, the PT team will give me the full list of what’s in and what’s out.
I’ll be honest — that messed with my head a bit. Part of me is thinking: what’s the benefit besides hopefully getting rid of the pain? And if I’m not supposed to do a bunch of the shoulder-related lifts, then what does “back to full capacity” really mean? I can barely do some of those lifts right now anyway, and when I do it’s light, but at least I still feel like I’m training. The idea that after surgery I might lose even that is bothering me.

So yeah… I’m kind of in two minds about the whole thing right now.

Recovery basics he gave me:
  • Sling for 6 weeks
  • Ice every 2–3 hours for 30 minutes (For first week or till swelling subsides)
  • Sleep sitting up (which I’m not excited about to avoid injuring the shoulder)
On the positive side:
  • I should be able to get back to the gym in 7–10 days just to walk on the treadmill
  • And possibly start lower body training around the 3-week mark

Tomorrow is go-time. I’ll update once I’m out and on the other side.

Good to see you share this :D @rayray01 but in the future dont use ai to write things just talk to us the way you do, we need to talk to you not a chatbot :D even if you use it to clean up writing
I think this will be a good thing we can slowly step by step work towards recovery and training.

@rayray01 shoulder injuries are super common. i had it too but mine did not require surgery, just a lot of rehab and rest. unfortunately i was told if i kept training heavy it would tear and i would need surgery so i had to give up powerliting.

Bros, this is a good setup on this recovery stack. I got a lot of respect for it. maybe the only one you missing KLV, which will be in KLOW stack @rayray01

Got back home now, surgery went well, noted just have ice every couple of hours and do the pendulum with am ever few hours, the nerve block is a life saver so far not feeling any discomfort, but as it wears off over the next 12 hours am sure I will find out the pain levels.

I keep forgetting that my right arm is a complete noodle at this point, every time I took it out to do the pendulum it just flops around as if it v is not even attached to me anymore. Waiting for the effects of anesthesia to wear off as well.

For post op have given me oxycodone..

Thanks, I will order it today, should have it in couple of days. Challenge will be learning to do this with arm, as an draw it in the syringe part, pinning would be fine

Now definitely comes the hard part. @rayray01 the rest and rehab is going to be crucial. Hopefully, you'll be back like new pretty soon.

Agreed, patience, being mindful at all times, and follow directions to the T during physical therapy will be the key. I am determined to bounce back strong and hopefully sooner than later... but not rushing anything.

@rayray01 should you inject the peptides after surgery or wait a little bit? that is a good question. some people say to wait a bit before you start using them and let the healing process work

Hmm oops!!!! A little late for today!!! But will hold from tomorrow onwards for sure. I guess I was ready for the bpc/tb to get in there start it's magic... lol

Oh yeah for sure I always pin the peptides in my belly subconscious fat. And deca was upper glutes.
I'm very curious to see how you recover, and you're not the first person who has had this issue with their shoulders and who have tried to recover through it. You will get back to where you were at, trust me. @rayray01

Please be careful with the percocet. Take PRN as needed.

Oh yea I have a very high tolerance for pain even when I got them for my kidney removal surgery, I had more than half left over, that i ended up throwing away once they expired. I haven't even picked them up from the pharmacy for this procedure. Will get them if I absolutely feel the need for them...

I stayed active post surgery, completed all my daily activities, without much issues, granted it took a bit longer because my right arm was just numb doe to nerve block but at te end of itnincompelted what I wanted to do. Even so much so that I am feeling encouraged en I ugh to go to the gym tomorrow morning and start my walking on the treadmill just no push, pull or anything with the right arm might try to see how lower body excersizes go. Let's see how it goes at the minimum will do the treadmill walk.

happy to hear :d can you walk? @rayray01

Don't see why I won't be able to walk. Been walking all day, will start slowly on the treadmill no speed walking or anything hate want to keep my habit of going to the gym. Will skip sauna for about 2 weeks till the stitches after our atleast.

The doc said to give it a week, but I think I will try tomorrow my fat % has gone up 1% in about a week of no proper excersizing and up 4 lbs... need to stay active somehow.

I wish to really use this time to focus on lower body workout and really grow the size of my thighs and calc, so much so that i want to add like good 5 lbs combined of muscle in legs. Would progressive lifting get me there and trt test get me there?? Or waking am hours everyday would be counter productive to it. Please advise or up the deca a read but to assist with that. @LevButlerov

Surgery

Type of Surgery - SHOULDER ARTHROSCOPY WITH ROTATOR CUFF REPAIR, SHOULDER ARTHROSCOPY EXTENSIVE DEBRIDEMENT

Post Surgery Gameplan (This is on top of whatever Doctor recommendation)

Start all my supplements again with normal dosage (Non Cycle Dosage)
BPC157/ TB500 - Daily Injection
GHK- Cu - Daily
Ipamorelin - Daily
Reta - Weekly
AOD 9604 - Daily
Deca - 50mg/weekly
Testosterone - TRT Dosage (120mg/weekly)

@rayray01 Lets go bro! Wishing you a speedy recovery!

I wish to really use this time to focus on lower body workout and really grow the size of my thighs and calf muscles, so much so that i want to add like good 5 lbs combined of muscle in legs. Would progressive lifting get me there along with trt test?? walking an hours everyday hopefully would not be counter productive to it. Please advise or should we up the deca a bit a read to assist with that name it a lower body bulk cycle. What are c your thoughts. @LevButlerov

its possible lets get there when you're more recovered :D I think after recovery +5lbs is hard, we should go slow 8 weeks and then ramp up @rayray01

Don't see why I won't be able to walk. Been walking all day, will start slowly on the treadmill no speed walking or anything hate want to keep my habit of going to the gym. Will skip sauna for about 2 weeks till the stitches after our atleast.

The doc said to give it a week, but I think I will try tomorrow my fat % has gone up 1% in about a week of no proper excersizing and up 4 lbs... need to stay active somehow.
lets walk on treadmill after a week, and get bone broth ed

Ok let's do that. We'll if was using 5lbs as an example, but what i meant was need to put on some muscles on my skinny twigs for legs. Lol. I was c thick to start back up on clen to ensure I am keeping my fat burn rate high. Same as before 20mcg 2 weeks on and 2 weeks off, between that and reta I should be able to avoid gaining any weight over v the course of next 6 months. @LevButlerov .

dont use clen in recovery it can be an issue

The only thing you should be taking is low dose TRT like 125mgs of test thats all @rayray01
This is what i had planned. @LevButlerov

Ipamorelin - Daily

This is perfect, 125 or 120 depends mg/ml on the test bottle @rayray01
AOD is not that crucial but you can keep.

Thanks, I will order it today, should have it in couple of days. Challenge will be learning to do this with arm, as an draw it in the syringe part, pinning would be fine
Bros, you're going to come back like a warrior. We believe in you

Agreed, patience, being mindful at all times, and follow directions to the T during physical therapy will be the key. I am determined to bounce back strong and hopefully sooner than later... but not rushing anything.
That is a great approach to have and very smart. I'm glad you have that mentality.

@rayray01 way to break it down force man really shows what you’re going through. I think I may have the same issue going on. I just don’t wanna go have surgery on it. Can’t wait to see you push through this.

IMO those kits arent really accurate. Best bet if you plan to test compounds send them out for testing. You are looking at primo?
yea i just ordered 8 vials of it from pharmaqo labs, through naps, since it has been in short supply, wanna validate it is what they are claiming it to be, i asked for coa from them they shared one from 2022 and 2024, not from the current batch. hence wanna check at least 1 vial. i ordered 5 trens A & E mix, 5 masteron P& E mix, 3 winstrol AQ (all injectables) so whenever i am ready and recovered i can start any cycle as per advice from lev. already have 3 eq, 3 deca, 6 vials of test cyp on hand, so i will be able to run any combo advised without delay or ordering and waiting. (kind of an overkill) but i have this fear of stuff running out...lol @Warthog61

Tren??? Thats not a typo? I believe pharmaqo runs batch numbers so you could see if they have updated HPLC's direct from them or email naps for an updated one. Its a great choice but not sure of tren in the future for you, wouldnt be my 1st choice. Seems a little aggressive with a solitary kidney, but i'm not one for accurate advice. LISTEN TO @LevButlerov @HarleyGuy and other vets on this, i know you are in recovery which will be a long road ahead but stay positive. I also know you a very eager person and you need to be smart about all of this.

There isnt much you can't get done on a low dose of test, especially with your size

I ordered it just to have it in stock will only do what @LevButlerov will advise. But I would like to try it one day. Yea first gotta get to a point to where I can workout full body, hopefully in 3 months time will able do another recomp cycle.

I did ask naps and they shared 2022 and 2024 test results nothing from this batch but will look into what you shared about batch number and validating through pharmaco. @Warthog61
looking good :D @rayray01

no need, how do you figure you have low salt?
My.labs came back with low sodium, hence asked about this. @levbuterlov

Ok i am glad I asked. I will waitand stick to the original gameplan.
I'm glad too, go easy on the sauce and all compounds. Add in some extra collagen too and go easy for 8-10 weeks. Marathon, not a sprint. What matters is how you feel and look in a years' time and not next month.

Yes sir.. that is the gameplan.

Yesterday diet and excersize
Good macros and calories when you're not moving as much.

Have you got the collagen peptide powder yet?

For the leg extension machine are you not hanging on to one side, same with leg press?

Yes I have submitted a ticket and DM Jason as well for this. I was so excited when I received it that all my primo are here but when I opened it, i was like what is this? Even the tracking number on the box does not match any tracking number listed at Naps for my orders marked shipped. I feel bad for the person whose order got delivered to me..

The only thing common between this order and mine is one tren E amd one Nandrolone D, rest all is different.

You’re going to run tren? I’m curious about that

Yesterday's diet and excersize
Good to see you're choosing exercises that are working around your injury and you're not pushing the envelope.

How is the shoulder feeling? Pain levels?

Oh lower body only for next 3-4 months is all I see in my future... lol till ky physical therapy is completed and the doc say I can start lifting with right side shoulder. Hopefully all these legs days add a pound or two on my thighs

Yesterday's diet and excersize
This is too early, don't do ANY exercises minimum 10 days post surgery, you are causing inflammation and limiting recovery, just walk please @rayray01

You got it sor. So i had already worked out in the morning today, but won't be doing so.from tomorrow.
sure keep us posted :D and add bone broth ED

Today's activity and diet. Calories burned is reporting low because I forgot to wear my fitbit for like first 3-4hours of the day so didn't count my walking at the gym.

Good day, but add 1 avocado ed from now on @rayray01

Yes sir started bone broth today, still waiting collagen peptide will start when it gets here.
Great job getting the collagen peptides have you decided on dose? I think you'll wanna keep it high for a while. I take 20g ED.

No I have not whatever you recommend will roll with that...
If you have the discipline add one scoop with each meal. You're around 3-4 meals a day so there's your 15-20g.

Each meal, slightly confused the collagen peptide is a powder, so would i not drink it similarly as I do protien shake, or you are saying to add a scoop of it with my protein drink?? Please advise. @HarleyGuy

Protein drink = add scoop
With each meal = swirl in with some water and chug

There's your 20g.
