What Happened

About 2 months ago I had a pretty solid off on my Beta 350 out in the bush. The bike came down and landed right on my foot/ankle. Hurt like hell. Got it scanned straight away, no fractures thankfully but the scan showed soft tissue damage (ligaments/tendons around the ankle).

It's been about 8 weeks now and while it's improved, it's still not 100%. There's still some stiffness and discomfort especially in the mornings and when I'm pushing heavy on leg days. I can train through it but it's definitely limiting me on squats, lunges and anything that puts load through that ankle.

So I've decided to run a BPC-157 and TB-500 protocol for 8 weeks to see if it can get me over that last hurdle and fully heal this thing up.

Wednesday 8 April 2026 — Update (Lower A session from Mon 7 April)

Hit Lower A last night on the Upper/Lower Hypertrophy split. Legs are absolutely destroyed today 😂

Training (63 min) — weights per side, excluding bar:

Barbell Full Squat — 10kg x6, 20kg x6, 35kg x6, 50kg x6 — Pain: 1/5 | Pump: 4/5 | Workload: 4/5
Barbell Romanian Deadlift — 20kg x8, 45kg x8, 60kg x8 — Pain: 1/5 | Pump: 4/5 | Workload: 4/5
Lever Seated Leg Press — 120kg x10, 143kg x10, 165kg x10, 188kg x5 — Pain: 3/5 | Pump: 4/5 | Workload: 4/5
Smith Calf Raise — 109kg x20 x4 sets — Pain: 2/5 | Pump: 4/5 | Workload: 4/5
3/4 Sit-up — BW x20 x4 sets (superset with calves)

Skipped Leg Curl and Hip Thrust — still easing back into leg volume.

Leg press had the most discomfort in the ankle (3/5 pain) which makes sense with the foot position and load angle. Squats and RDLs were basically pain free which is a good sign.

Ankle Update:
Haven't started the BPC-157/TB-500 injections yet — waiting on bloods next week for a baseline before I start pinning. Want clean before/after data so I can properly track the recovery for the log.

How I'm Feeling:
Energy: 7/10 (leg DOMS already setting in)
Mood: 9/10

Haven’t ridden dirt for a few years now switched over to circuit racing on the road bikes. Wouldn’t mind getting another one for some fire trails where I’m at in QLD. Good cardio training too 😂

Haven’t ridden dirt for a few years now switched over to circuit racing on the road bikes. Wouldn’t mind getting another one for some fire trails where I’m at in QLD. Good cardio training too 😂

Bro the cardio its unreal on a dirt, I'm more like a hard enduro kinda guy, I usually burn about 6k cal when I go out to ride for a full day.

Thursday 9 April - Rest Day Update (Wed 8 Apr)

Rest day yesterday. No training, just recovery and food.

Ankle update - no pain at all as a result from Tuesday's leg day which is really encouraging. Squats and RDLs didn't leave any lasting soreness. That said, still very limited range of motion and I get pain when I flex my toes up towards my shin (dorsiflexion). Day to day walking around is fine though. Still waiting on my bloods before I start the BPC/TB protocol so I've got a proper baseline to compare against.

Slept 12 hours last night, went down at 10pm and didn't wake up until 10am this morning. Body clearly needed it. Feeling great today, energy is 9/10. Stuck to the meal plan, nothing exciting to report on the food front.

The new Upper/Lower split is treating me well so far. Getting used to the different structure compared to the PPL but enjoying the extra recovery between sessions.

Saturday 11 April - Update (Lower B from Fri 10 Apr)

Lower B yesterday, 6 exercises, about an hour session. Switched from back squats to front squats for a change and that was interesting.

Training:
- BB Front Squat: 15x10, 20x10, 30x10, 40x6 (per side)
- Lever Seated Leg Extension: 77x12, 90x12, 97x12, 104x12
- DB Lunge: 25x12, 30x12, 30x12 (per hand)
- Lever Seated Leg Curl: 70x12, 77x12, 84x12
- Cable Donkey Kickback: 18.1x12 x3 (each side)
- Lever Seated Calf Press: 109kg x20 x4

Ankle: Calf press still flares it up (3/5 pain), the dorsiflexion under load is where it complains the most. Front squats were manageable at 2/5, leg extensions and curls no issues at all. Lunge was a bit of a struggle. Day to day walking is fine.

Wrist: Front squats exposed an old tendonitis I used to get, mainly in my keyboard hand (right). Haven't front squatted in a while and it let me know about it. Since I'm starting BPC from @Prymal on Monday I'm planning to split the dose, half SubQ on the ankle and half on the wrist, and log improvements on both.

Protocol: BPC-157 + TB-500 from @Prymal starting Monday once baseline bloods are back.

Subjective: Energy 8/10, Mood 8/10, Libido 8/10

Im usually a pretty tough cookie but yeah it's annoying, I can go about the whole day pretty much without feeling anything but there are some specific movements that triggers it :(

I'm going to sleep with the sleeves tonight to see if it helps.

Did a research onto it and for whoever is also curious about it:

A PMC meta-analysis https://pmc.ncbi.nlm.nih.gov/articles/PMC8364035/ of RCTs on photobiomodulation for tendinopathy showed 40-60% pain reduction and improved function after 4-8 weeks.
An LED-specific study https://pmc.ncbi.nlm.nih.gov/articles/PMC10351370/ on tenosynovitis (NSAID-refractory patients) showed significant improvements in pain and stiffness at weeks 2, 4, and 8.

Wavelengths

- 850nm (near-infrared) - best for deep tendon penetration, stimulates tenocytes (cells that rebuild tendon matrix)
- 660nm (red) - good secondary for superficial tendons like wrist extensors and surface inflammation
- Ideally a device that does both

Protocol

- 10-15 min per site, 4-6 inches from skin
- 5x per week, minimum 6 weeks
- Expect pain reduction in 2-3 weeks, functional improvement 5-7 weeks, actual tendon remodelling 8-12 weeks

Meals: Mostly on plan, skipped the arvo oats. Smashed 3 hot dogs for dinner as a cheat meal, took me til midnight to fall asleep lol, sodium + heavy protein that close to bed = no bueno.

Ankle: No aggravation carrying over from the leg session, walking fine day to day. Same limited ROM on dorsiflexion but no active pain at rest.

Wrist: The flare from front squats has settled with rest.

Today: Slept 8hrs great quality after finally getting down. Energy 7/10, Mood 7/10, Libido 7/10.

Back into it tomorrow with Upper A.

Mon 13 Apr 2026 - BPC-157 + TB-500 Kickoff Day

Finally got the peps from @Prymal reconstituted and pinned today. Rest day yesterday but plenty going on today.

Reconstitution
- BPC-157: 10mg vial + 2mL BAC water = 5000mcg/mL (so 250mcg = 5u on a 50u slin pin)
- TB-500: 10mg vial + 2mL BAC water = 5000mcg/mL (2.5mg = full 50u pin)

I personally just pin BPC in my stomach. Its a water based peptide, so it goes systemic really fast. I dont think it actually needs to be pinned locally, otherwise it would have to be done IM.

Tuesday 14 Apr Update (Training update from Mon 13 Apr)

Day 1 of the full BPC + TB-500 protocol stacked. AM ankle shot BPC 250mcg SubQ clean, zero PIP. PM wrist shot BPC 250mcg SubQ, also clean. TB-500 loading dose 2.5mg into the belly, no head rush or flush that some guys report on the loader. Noticed a small red rash at the belly site this morning, could just be a TB injection site reaction, will confirm next pin.

As for the wrist injection site here is what I did, since my skin has fuck all fat in it I am not confident to just injecting the 4/5mm needle in 90 degree angle, to get a good pinch and still able to inject I used a clothesline pin to hold the pinch so I can inject under the skin.

This side is actually where the pain is coming from (de quervain's tenosynovitis) I have a lot of vein and shit over that area so the pinch method is great.

I've been pinning just near the Achilles tendon for plantar facilities its not a fun spot to pin

Yeah that Achilles area is brutal, almost no fat cushion to work with. Did you find the plantar fasciitis responded well to the local dosing? Asking because my wrist spot is the same story, barely any padding, so I'm hoping the local approach pays off on both stubborn tendon/ligament spots.

Fair point mate, the water based systemic argument comes up all the time. My read on it is that even if BPC circulates systemically, pinning close to the site concentrates the dose in the target tissue before it distributes out. The animal studies that showed tendon and ligament regen used both IP and local pins, so I figure SubQ nearby gives me a better shot on stubborn spots than going belly. Happy to be wrong if my ankle hasn't moved in a few weeks, we'll see haha.

Day 2 of the BPC loading stack done, zero PIP at either site. BPC ankle AM + wrist PM (5u SubQ each), clean pins again.

Training: Lower A (1h 29m)
  • Back Squat 4x6 @ 15/25/40/50kg (ramped, kept top set conservative for the ankle)
  • Leg Press 4x10 @ 120/143/165/188kg
  • Bulgarian Split Squats 4x8 @ 20/25/30/35kg per side (subbed for RDL by mistake haha)
  • Leg Curl skipped, machine was taken the whole session
  • Hip Thrust 4x8 @ 25/55/70/80kg
  • Smith Calf Raise 4x20 @ 109kg
  • 3/4 Sit-ups 4x15 BW

Ankle: 1/10 on squats, 2/10 on the calf raise. Day 2 of BPC + TB loading already pulling its weight, the ankle is tracking better than I expected.
Wrist: 4/10 through the session, annoying but not debilitating. Looking forward to the wrist PM pins stacking up.
Sleep: 7/10, down at 10:30pm up at 8:30am, few wake-ups in between.

Tuesday 14 Apr Update (Training update from Mon 13 Apr)

Day 1 of the full BPC + TB-500 protocol stacked. AM ankle shot BPC 250mcg SubQ clean, zero PIP. PM wrist shot BPC 250mcg SubQ, also clean. TB-500 loading dose 2.5mg into the belly, no head rush or flush that some guys report on the loader. Noticed a small red rash at the belly site this morning, could just be a TB injection site reaction, will confirm next pin.

As for the wrist injection site here is what I did, since my skin has fuck all fat in it I am not confident to just injecting the 4/5mm needle in 90 degree angle, to get a good pinch and still able to inject I used a clothesline pin to hold the pinch so I can inject under the skin.

This side is actually where the pain is coming from (de quervain's tenosynovitis) I have a lot of vein and shit over that area so the pinch method is great.

I think this is the first time I've seen someone use a closed pin like that to do their injection. That's pretty cool stuff, man. Thanks for sharing this; it's a valuable thing. @bss

Hell yeah! Especially now that I am "natural" only on very small (80mg/week) dosage of T I can't do too much volume.

Wed 15 Apr Update (Training update from Tue 14 Apr)

Day 2 of the BPC loading stack done, zero PIP at either site. BPC ankle AM + wrist PM (5u SubQ each), clean pins again.

Training: Lower A (1h 29m)
  • Back Squat 4x6 @ 15/25/40/50kg (ramped, kept top set conservative for the ankle)
  • Leg Press 4x10 @ 120/143/165/188kg
  • Bulgarian Split Squats 4x8 @ 20/25/30/35kg per side (subbed for RDL by mistake haha)
  • Leg Curl skipped, machine was taken the whole session
  • Hip Thrust 4x8 @ 25/55/70/80kg
  • Smith Calf Raise 4x20 @ 109kg
  • 3/4 Sit-ups 4x15 BW

Ankle: 1/10 on squats, 2/10 on the calf raise. Day 2 of BPC + TB loading already pulling its weight, the ankle is tracking better than I expected.
Wrist: 4/10 through the session, annoying but not debilitating. Looking forward to the wrist PM pins stacking up.
Sleep: 7/10, down at 10:30pm up at 8:30am, few wake-ups in between.

Today (Wed 15 Apr)
  • Rest day
  • BPC ankle AM done, BPC wrist PM tonight
  • No TB today (next loading pin Thu)
