Blood Work Hematocrit .55 on 225mg Test Enanthate - fast rebound after venesection

Currently running 235 mg/week testosterone (split 3x weekly). Hematocrit has come back at 0.55.

Got a venesection and 19 days later I had my bloods checked and .55 hematocrit.

I’ve previously run lower doses 9150 mg/week) without any hematocrit issues.

Other context
* well hydrated before bloods
* Eating clean and in surplus
*On trt several years
* Sleep apnea well managed

Plan at this stage is to drop back to 150mg/week, organise another venesection, and retest after stabilising.

Interested to hear from others who’ve had a similar issue or any suggestions.

You can drop the doses back down but that's hard to say. We don't know enough about you.
can you share the actual blood work paper to start? we need to see all ranges please :D

Can we start by you taking screen shots of your bloodwork (with info blurred out) so we can get a full grasp on what's going on?
If you had a venesection for purposes of lowering your RBC's or iron levels it's possible you may have needed to have done it sooner and ought to keep going until it's in range. I'd post your bloodwork for us just to get this started. Having a raised hematocrit level is common during TRT+.

High hematocrit seems to be a recurring issue for me.

I don’t have my latest bloods yet as they’re with my GP, but here are results from January 2026 for context.

At that time I was:
• ~150 mg/week testosterone
• 2 IU HGH daily (reflected in IGF-1)
• 30 mg/day Anavar (ran for ~12 weeks)

Currently:
• ~225 mg/week testosterone
• 2 IU HGH daily



Bloods (Jan 2026)

Hormones
• Prolactin: 144 mIU/L (ref <300)
• LH: <1 IU/L (1–10)
• FSH: <1 IU/L (1–10)
• Oestradiol: 121 pmol/L (<150)
• Progesterone: 1 nmol/L (<3)
• Testosterone: 24 nmol/L (10–33)
• Free Testosterone (calc): 631 pmol/L (150–700)
• SHBG: 22 nmol/L (13–71)

Adrenal
• Cortisol: 360 nmol/L

Full Blood Count
• Haemoglobin: 194 g/L (HIGH)
• Red Cell Count: 6.4 x10¹²/L (HIGH)
• Haematocrit: 0.55 (HIGH)
• MCV: 87 fL

So you were in range on just T only. The difference I see is the anavar then for this most recent test. It's possible the anavar played a role unless you were running anything else? EQ is notorious for high levels seen here for example. Correct me if I'm wrong but it seems like anavar may be the culprit (if it was real anavar and not dbol).

Good summary this clears it up better.

You can start by taking high dose fish oils and vitamin D. Vitamin E as well can help. I'd cut out red meats too and add in chicken breast and fish for protein with grapefruit which can help lower hematocrit.
 
These are my most recent bloods from my gp. This is not a false test. I've got plenty of evidence I respond like crazy to testosterone. Appreciate the advice so far and will give everything a go. I'm still going to go back to trt dose. Hoping to get into a position where I get a referral for regular venesection if I want to go higher. Still..... Back to .55 after only a few weeks... And that's after two venesections. The first lot of bloods here is after a half venesection. Got clotted and they only took about 250ml. I got the bloods done only a week after the first venesection. You can see it dropped a fair bit.

I waited a bit longer to get the results after the second venesectio. The got the full 500ml that time. Around three weeks later I got the blood test. Somehow my body managed to get me back to .55....

Haemoglobin is what you want to be focusing on, that is the more reliable indicator of whether or not your blood is thick and putting you at risk of clots.

This is better but not perfect. Can you take a photo of the actually bloods paper to upload? Hemoglobin is right on the cusp of high range.

The thing here is also long term food and training and cardio and TRT in general, a lot of this we are discussing here is guess work you understand right? :D @Gonad
usually if you bounce back to 0.55 that fast, you’re a heavy responder and dose, pin schedule, sleep apnea, and BP control all matter, but we can’t guide properly you without your day to day context.
But I did look at all the bloods here, it looks like true test driven erythrocytosis, not just dehydration, because you were 0.52 even after the partial venesection and then back to 0.55 with Hb180 a few weeks later.
You also already hit 0.55 back in January on around 150mg right? with 2 IU HGH and Anavar, so this is probably just how your body responds and 225mg is too much for your bloods tbh
My take is go back to true TRT, keep sleep apnea and BP fully checked, and get ferritin and iron looked at because this looks like high sensitivity to androgens, not a one off bad test. This is another reason we need a log for details.

Get a TRT log up now with your exact weekly dose and pin days, bodyweight trend, BP readings, sleep quality, cardio, and dates of venesection, so the EVO family can see the pattern and help you keep hematocrit under control. 15min to start the TRT Training log :D

  • Other guys here run logs all the time and there’s no privacy issue, nobody is asking for your name or anything personal, it is just training, diet, and how the compound feels.
  • The feedback you get is also way better because people can follow along and spot things you might miss. It also helps the next guy who is thinking about running the same compound so you are giving back while you learn.
  • Even if you think you might not stick with it, just starting the log makes it easier to come back and add quick notes. That way you get more out of what you are running instead of keeping it all in your head.

To really guide you we need more info from you:
  • Please share your diet, training, cardio, sleep, supplements etc log with us in a NEW thread, full log journal so our guidance is based on facts you provide.
  • It will take 15-20 minutes max.

Dropped back to normal TRT as of yesterday.

Got more bloods to post and can go into a lot more detail. I’ll start a proper log this weekend.

Bit hesitant on posting a photo as I’m no Greek god like some of you blokes… but whatever, I’ll be brave.

Sneak preview: I’ve got bloodwork showing high test with hematocrit still in range.

Pinning Mon, Wed, Fri.
Eating in a surplus. Use myfitnesspal to log daily.
Running a heap of supps which I’ll list in the log.

Also have a body comp report showing weight and body fat are in a good place.

My suspicion is I pushed things too soon after my Anavar run and didn’t cruise long enough before increasing test again.

If I were doing this properly from scratch, I’d cruise until both HCT and test are fully back in range, then slowly increase test and run bloods every 5 weeks to find my limit.

End of the day, probably a mix of my own dumb decisions and genetics. I’ve also got a gene that predisposes me to higher iron, so I’ll share that as well.

No worries brother we don't judge and we've all been there, I was 270lbs and posted the pics of myself in my 2025 log. I've 80lbs since I peaked at 270. I know you're brave you can do it bro it will help us help you. You got this!
