High blood pressure on test enanthate 120 mg/week – looking for advice

Hi everyone :)

I’m a 29-year-old male in good physical shape. I’ve been active my entire life. For the past two years I’ve been boxing, which involves a lot of cardio, and before that I trained at the gym for around 10 years. I’m muscular, weigh 89 kg (196 lbs), and I’m 182 cm tall (about 6 feet).


Exactly one year ago, I decided to try Sustanon for the first time. The dose was 175 mg once per week. Before starting, I had full blood work done and everything looked good (I’ll attach the results). A few weeks before starting Sustanon, I also quit nicotine pouches after more than 15 years of use


The first two weeks went well and I felt fine. During the third week, I decided to buy a blood pressure monitor—and that’s when things started to go downhill.


Even though I was doing cardio, eating relatively clean, avoiding caffeine and alcohol, and staying well hydrated, my blood pressure readings were consistently high. The monitor often showed numbers like 148/78 or even 156/73. Around the same time, I also started experiencing very strong panic attacks.


Because of this, I lowered the dose to 150 mg per week. Occasionally my blood pressure would drop to around 135/78, which gave me some temporary peace of mind, but overall I couldn’t get it under control.


Eventually, I got sick—possibly the flu or something similar—and was ill for about two weeks with a very high fever. When I went to see a doctor, my CRP (inflammation marker) levels were about 10 times higher than normal. After that, I decided to stop using Sustanon completely and stayed off for a full year.


In total, I only used Sustanon for six weeks (six injections).


Current situation


I recently decided to start again, but this time I didn’t do full blood work beforehand. I only managed to get partial labs done, and honestly, I was in a bit of a rush to begin.


This time I used testosterone enanthate, pinned twice per week, with a total weekly dose of 120 mg.


The first 10 days were extremely tough. Sleeping was almost impossible. There were days when I went to work having not slept at all, and others where I managed only a few hours—or sometimes just 30 minutes—of sleep. After the second week, things improved slightly.


I eventually adjusted my injection timing to 6:00 AM, twice per week, which helped to some extent. Sleep was still disrupted, but mostly on the day after the injection. Falling asleep on those days was very difficult—I simply had way too much energy.


Despite the sleep issues, I actually felt quite good overall. During the first two weeks, my blood pressure stayed completely normal.


However, during the third week, my blood pressure suddenly started to rise again. Readings jumped to around 156/80, 144/79, and 152/75, seemingly out of nowhere. It didn’t matter whether I measured in the morning or in the evening—the numbers were consistently elevated.


That was concerning enough that I decided to stop during the fourth week.


I’ve now been off for 25 days, and I really want to continue, but I’m honestly worried that my blood pressure will spike again to those same levels. At the moment, my blood pressure is still slightly elevated, around 132/75, but that’s nothing compared to what it was while on testosterone. My normal baseline blood pressure is usually 120/70 or lower.


I still don’t use nicotine pouches, and aside from this issue, I feel healthy.


I would truly appreciate it if anyone could share insights, experiences, or ask follow-up questions that might help me figure out a solution and possibly try again.

5mg of Cialis is pretty good at lowering BP.

Telmisartan is another good option. Start at 20mg and see how you go.

Thank you for the response. Yes, I understand that I can take blood pressure medication. However, what I don’t understand is why my blood pressure rises to such high levels even on TRT doses. What are the underlying mechanisms or reasons for this happening at relatively low or replacement-level testosterone doses?

Did weight jump up when you started the test? Typically if estrogen rises you will put on water weight which will elevate BP. I don't see estrogen results on your bloodwork so just guessing here.
Your test levels prior to the sustanon looked good and it looks like you never got your levels back to pre sus after. Have you put on fat? High BF will lower test levels and raise estrogen conversion.

Actually, my body weight went down slightly or stayed the same. I haven’t checked estrogen levels, but is it even possible for estrogen to rise with such a small dose and in such a short timeframe—around 20 days? My weight went up right after I quit testosterone. I don’t think I have high body fat. Maybe I just started eating a bit more after starting, and my body weight went up slightly.

I felt emotional about one week after my last injection, but otherwise I didn’t notice any signs of high estrogen.

I’m also not sure how to proceed from here. If I go for blood tests a month after stopping testosterone, will they still provide any valuable information?
They won’t help you now. With the info provided this is all guessing.

More than likely your e2 has spiked and caused a bunch of water retention and high BP, some people get quite high e2 on modest doses - myself included.

High BP on test is almost always fluid/e2 related, that said telmisartan will blunt that well as it forces the body to dump the fluid.

If your introducing hormones you need to run full panel of bloods
 
BP can go up even on lower Test doses because testosterone affects fluid retention, red blood cells and vascular tone. Many guys see higher readings just from those changes. Best bet is more frequent blood pressure checks and making sure estrogen is controlled too

I’ve been thinking about how to move forward from here. It’s been almost Four weeks since my last injection, and based on how I’m feeling, my testosterone levels seem to be very low at the moment.
If I were to get an E2 blocker and telmisartan, would it be safe to continue from where I left off, or would you recommend a different approach ?

Thank you for the message. I do plan to start LOG, but first I’m trying to figure out whether it’s possible to safely jump back onto my mini cycle and address the blood pressure issues with an E2 blocker and telmisartan.
The idea would be to stabilize things, manage estrogen and blood pressure, and then get blood work done in about four weeks to see where everything stands.

Does this sound like a really bad or irresponsible approach 😁?

Start again with test, monitor your blood pressure, get bloods done after 4-6 weeks for your e2, I am one of these people that truly thinks telmisartan’s positives outweigh any negative for majority of people so I would say adding 20-40mg of that would be fine to run along side your TRT.

Dont add an AI before seeing some bloods though, wait for your test levels to settle, get your bloods done and then assess whether or not its needed - post your bloods here and we can help.

I bet you anything if he pulled his bloods he’d be well over the ref range on his lower dose.

I agree here too bro, though i dont feel all his trouble is worth it when he says he is already healthy and fit. If his e2 is significantly high then small dose a.i might not cut it and why bither pushing bigger doses of a.i for trt dose cycle.

If he is dead keen to continue, as you mentioned run it back again with telmi on hand and get ready to pull bloods again.

I’ve been thinking the same thing about is this worth it. This photo was taken before I ever tried steroids, so I’d say I’m quite happy with my physique. I weigh 90 kg here. I’ve always been able to build muscle easily, and it feels like after just a few weeks of training I can get back to this kind of “peak” condition.
The real issue is something else. I’ve completely lost my drive and motivation. My sex drive has also dropped almost entirely after I first quit nicotine pouches and then stopped using Sustanon after my 6week trial.(Libido Never came back) .
Based on blood tests, my testosterone levels never returned to the same level after the Sustanon experiment — or it could be partly explained by the fact that the later blood tests were taken around 11:00 a.m., which may affect the results.

I do have a girlfriend, and this situation obviously isn’t great, since my libido is very low. I still get morning erections, though, so that’s there.

I agree here too bro, though i dont feel all his trouble is worth it when he says he is already healthy and fit. If his e2 is significantly high then small dose a.i might not cut it and why bither pushing bigger doses of a.i for trt dose cycle.

If he is dead keen to continue, as you mentioned run it back again with telmi on hand and get ready to pull bloods again.
I have a deviated septum, which means I usually sleep with my mouth open. Do you think this could be related to my blood pressure issues at this point?
It also seems that the timing matches when my blood pressure spikes occurred during both my first and second "cycles"
 
Would need to see where estrogen is at before you throw an a.i into the mix but it does sound like this could be the cause.

To get a better picture would really have to start test dose again and be ready to pull bloods as soon as bp gets high to check e2.
Have Telmisartin on hand, but ask yourself bro is all this even worth it to run a trt dose of test when your already fit health and in good shape? Imo no way
I would consider running a much higher dose, for example around 300 mg, but my body doesn’t seem to tolerate even 100 mg. The original plan was to increase the dose gradually, but I never managed to get that far 😂

Before starting, I had full blood work done and everything looked good (I’ll attach the results). A few weeks before starting Sustanon, I also quit nicotine pouches after more than 15 years of use


The first two weeks went well and I felt fine. During the third week, I decided to buy a blood pressure monitor—and that’s when things started to go downhill.


Even though I was doing cardio, eating relatively clean, avoiding caffeine and alcohol, and staying well hydrated, my blood pressure readings were consistently high. The monitor often showed numbers like 148/78 or even 156/73. Around the same time, I also started experiencing very strong panic attacks.


Because of this, I lowered the dose to 150 mg per week. Occasionally my blood pressure would drop to around 135/78, which gave me some temporary peace of mind, but overall I couldn’t get it under control.


Eventually, I got sick—possibly the flu or something similar—and was ill for about two weeks with a very high fever. When I went to see a doctor, my CRP (inflammation marker) levels were about 10 times higher than normal. After that, I decided to stop using Sustanon completely and stayed off for a full year.


In total, I only used Sustanon for six weeks (six injections).


Current situation


I recently decided to start again, but this time I didn’t do full blood work beforehand. I only managed to get partial labs done, and honestly, I was in a bit of a rush to begin.


This time I used testosterone enanthate, pinned twice per week, with a total weekly dose of 120 mg.


The first 10 days were extremely tough. Sleeping was almost impossible. There were days when I went to work having not slept at all, and others where I managed only a few hours—or sometimes just 30 minutes—of sleep. After the second week, things improved slightly.


I eventually adjusted my injection timing to 6:00 AM, twice per week, which helped to some extent. Sleep was still disrupted, but mostly on the day after the injection. Falling asleep on those days was very difficult—I simply had way too much energy.


Despite the sleep issues, I actually felt quite good overall. During the first two weeks, my blood pressure stayed completely normal.


However, during the third week, my blood pressure suddenly started to rise again. Readings jumped to around 156/80, 144/79, and 152/75, seemingly out of nowhere. It didn’t matter whether I measured in the morning or in the evening—the numbers were consistently elevated.


That was concerning enough that I decided to stop during the fourth week.


I’ve now been off for 25 days, and I really want to continue, but I’m honestly worried that my blood pressure will spike again to those same levels. At the moment, my blood pressure is still slightly elevated, around 132/75, but that’s nothing compared to what it was while on testosterone. My normal baseline blood pressure is usually 120/70 or lower.


I still don’t use nicotine pouches, and aside from this issue, I feel healthy.


I would truly appreciate it if anyone could share insights, experiences, or ask follow-up questions that might help me figure out a solution and possibly try again.

The first would be nutrition and hydration (water intake and electrolytes). What does your food look like when blood pressure is high? Do you know your sodium and potassium intake? And how many L's of water do you drink each day?

The other is caffeine intake. In line with getting back on testosterone are you increasing your caffeine intake?

Just touching on sleep quality as well can you please give me a rundown on everything you do/ eat/ supplement/ inject within the couple of hours leading up to your sleep?

Sleep is fundamental to any physique or strength related goal so we wanna fix this up.

Thank you for the message. I do plan to start LOG, but first I’m trying to figure out whether it’s possible to safely jump back onto my mini cycle and address the blood pressure issues with an E2 blocker and telmisartan.
The idea would be to stabilize things, manage estrogen and blood pressure, and then get blood work done in about four weeks to see where everything stands.

Does this sound like a really bad or irresponsible approach 😁?

I don’t follow a strict diet, but I know what’s unhealthy and try to avoid it. Sometimes I eat too much sugar, especially if I smoke weed.

I drink around 2 liters of water per day
(3-4L if I train that day ).

I’ve noticed that while on testosterone, my urination increases and my appetite goes up.

I haven’t tracked electrolytes at all 🤔, and I don’t know my sodium or potassium intake 👀.
My meals usually consist of meat, some carbs, and salad. I can look for some example photos from my phone gallery.

Caffeine:
Normally, I drink about 4 large cups of coffee a day. When I started "TRT", I reduced it to 2 cups and stopped drinking after 2 PM. I noticed caffeine affects me much more strongly while on testosterone.

Supplements I use:
Zinc
Fish oil
Vitamin C
Biotin
Joint suppliment
Sometimes Ashwagandha

Evening:
Liquid magnesium

Injection timing:
I inject early in the morning on monday and thursday , around 6 AM before work. I noticed that injecting later makes me extremely energetic in the evening, and my eyes won’t close.

A few hours before sleep:
Gym and cardio around 5 PM (if I train later, I’m too amped up to sleep while on "TRT")

Dinner around 7–8 PM

9 PM: Liquid magnesium

10 PM: Check blood pressure
(usually was very high even in the mornings)

10:15 PM: If I feel super energetic and think I won’t sleep, I might take an antihistamine to calm down (usually doesn’t help)
Sometimes I smoke a little, hoping it will make me tired (works sometimes)
Might take melatonin (usually doesn’t help)

10.30- 11 PM: Trying to sleep
Wake up: 6 AM

Sleep issues:
My sleep has become extremely light and fragmented. Often I don’t feel rested at all. There have been days where I only slept 1–2 hours over two days, or not at all, and even though my body and mind are exhausted, I still have energy and my eyes won’t close. It’s extremely frustrating.

A lot of guys on here gave you some good tips, but the bottom line truth is, whether you use any type of hormones, whether you're doing a TRT dose above a TRT dose or below a TRT dose, you can have high blood pressure issues with or without steroids and with or without even a low dose of testosterone. Heart disease is a number one killer now among both men and women in North America. It's extremely common.

Good work on pulling the caffeine back

I'd recommend getting some more potassium into your diet! Try adding some of the following, and increase your water intake to 4L all days.

Potato
Tomato
Banana
Dried apricot
Avocado
Spinach
Heart salt

This should help drop your blood pressure 10-20 points.

Regarding sleep. That's a tricky one. Do you take any medication? Prescription or over the counter?

A lot of times your blood pressure will be high in the mornings, believe it or not, because you wake up very depleted and dehydrated, sometimes.
 
Bros, you should see my blood pressure on tren. It's through the roof. At the end of the day, it's good that you're keeping an eye on it. You don't want it to stay elevated for too long or you could end up having major issues. I use N2Guard. It really does help.

When I went to see a doctor, my CRP (inflammation marker) levels were about 10 times higher than normal. After that, I decided to stop using Sustanon completely and stayed off for a full year.


In total, I only used Sustanon for six weeks (six injections).


Current situation


I recently decided to start again, but this time I didn’t do full blood work beforehand. I only managed to get partial labs done, and honestly, I was in a bit of a rush to begin.


This time I used testosterone enanthate, pinned twice per week, with a total weekly dose of 120 mg.


The first 10 days were extremely tough. Sleeping was almost impossible. There were days when I went to work having not slept at all, and others where I managed only a few hours—or sometimes just 30 minutes—of sleep. After the second week, things improved slightly.


I eventually adjusted my injection timing to 6:00 AM, twice per week, which helped to some extent. Sleep was still disrupted, but mostly on the day after the injection. Falling asleep on those days was very difficult—I simply had way too much energy.


Despite the sleep issues, I actually felt quite good overall. During the first two weeks, my blood pressure stayed completely normal.


However, during the third week, my blood pressure suddenly started to rise again. Readings jumped to around 156/80, 144/79, and 152/75, seemingly out of nowhere. It didn’t matter whether I measured in the morning or in the evening—the numbers were consistently elevated.


That was concerning enough that I decided to stop during the fourth week.


I’ve now been off for 25 days, and I really want to continue, but I’m honestly worried that my blood pressure will spike again to those same levels. At the moment, my blood pressure is still slightly elevated, around 132/75, but that’s nothing compared to what it was while on testosterone. My normal baseline blood pressure is usually 120/70 or lower.


I still don’t use nicotine pouches, and aside from this issue, I feel healthy.


I would truly appreciate it if anyone could share insights, experiences, or ask follow-up questions that might help me figure out a solution and possibly try again.

We’re the same size. I’ve always been fit with good cardio too but once I hit 40 I started struggling with blood pressure and gear can raise it easily. Even my mom, who’s 69, was an athlete and works out every day to this day, very youthful and healthy with an uncommonly healthy diet, she is medicated for her blood pressure. Some of us are unlucky in that department.

We also both have a history of panic attacks, although mine aren’t debilitating and I express them differently, usually anger. The first time it completely exhausted me and I went to the clinic they said I needed to go to the emergency room and they couldn’t believe it had been going on for like 14 hours. I felt TERRIBLE.

If I was you, the first thing I would do is go to the doctor and get some real medicine. Get that blood pressure down first! Definitely consider leaving the gear alone, but if you choose to do it, keep checking like you have been. Get your blood work.

When I first felt the effects of testosterone — the mood, libido, and energy — I literally thought, “Ahh, this is what I need.”
It gave me a strong sense of mental confidence, a feeling that everything was okay, and that I could actually go far in life.

I know exactly how you feel. When you go to the doctor, yeah your cuff with you. They are notoriously unreliable. I myself have bought two cuffs that aren’t accurate and had to have them tried against the ones at the doctor. You’ll also find that you’re blood pressure may be suits a bit different from one to the other arm. I’ve never found a wrist cuff that was worth saving, they are are junk

Hmmmm yeah. Well. If you’re going to use it yours going to have to make sure you’ve got it handled. And get that bloodwork. Probably an estrogen thing.
