Second and onwards just the two sets (and for smaller muscles he says you can just do the top set). I personally think Mike Mentzer and to a lesser extent him, take it a bit too far. Bit of a setback unfortunately, busted my right elbow last monday doing heavy tricep extensions, seems to be worse then originally thought .
Have had a chat with coach trying to find exercises that wont aggregate it further while it heals.
Impacting all push/pull and arm workouts.
Its a bugger too was really building some momentum.
Icing daily, KLOW is in as well as HGH not sure what else can be done apart from resting it.

Elbows are a bitch cuz it's really tough to train around. Tricep movements see the worst on elbows when you're using a bar. The movement path isn't right for the joint. Hopefully it will heal up soon with all the peptides.
