Why Does My Belly Still Stick Out at 8.2% Fat? Need Help Understanding What's Going On

Hi everyone,

I’d really appreciate it if someone could help me understand what’s going on with my belly. Despite having very low body fat, it still pops out noticeably. I have no bloating, gas, or gut issues — it just looks like my abdomen is always protruding. I’ll be sharing Dexa scan reports and photos to back this up.

Background:

I was obese for over a decade, but over the last two years, I lost all the excess weight through diet and now weigh 62 kg. My latest DEXA scan shows:
  • Total body fat: 11.9%
  • Belly fat: 8.2%
  • Visceral fat: 134 g

So as you can see, my belly fat is very low. Yet, my abdomen still sticks out — even first thing in the morning, on an empty stomach after using the toilet. There’s no bloating or digestive discomfort. When I touch my belly, it feels like there’s a thin layer of skin, and underneath that, a sort of “balloon-like” layer pushing outward. It doesn't feel like typical fat.

When I consciously pull my belly in, it flattens — but of course, that’s not natural and only happens when I hold it in.

Throughout the day, especially after meals, it expands even more, and by the evening, it often looks like I have a belly again, despite being lean. Strangers would probably assume I still need to diet.

What I’m Trying to Understand:

  1. Why doesn’t my stomach lie flat like others at a similar body fat percentage?
  2. Could this be due to weak core muscles? Will strengthening my core help flatten it?
  3. Is this a case of stretched fascia or a weakened transverse abdominis that can’t hold the organs in?
  4. Could it be diastasis recti? I’ve done the finger test in a crunch position and the abs feel fully connected.
  5. Do I possibly need surgery, like a tummy tuck (abdominoplasty), to repair or tighten the muscles?
  6. Is there anything else I might be overlooking?
Any advice, insights, or feedback from those who’ve experienced something similar — or professionals with knowledge in this area — would be deeply appreciated. Thank you so much for taking the time to read and help!

6. Yeah. You’re probably just a little bloated. Almost everything is living their lives I. An almost constant state of bloat unless they are fasting. I’ve been blessed to have visible and flat abs since I was a kid, but if I eat the wrong food I look very similar to you, especially when I was younger and less muscular

bro i had a cousin like this. turn out he had a large worm in stomach. it was wrap around intestine was 8 foot long @DarkAthlete
 
Definitely start a log! Check out some of the logs on here. Some really great transformations. My log got me where I am right now with less than half the stuff I took to get where I was a year ago, only I had acne bad and a puffy face and worse bloodwork. You’re already lean, so you can just start making gains directly. You’ll get lots of great advice about food and training and anything else from all these super experienced dudes. I honestly feel like if I had found a thing like this to help me when I was much younger I could have been a reasonably successful body builder. Lots of positivity and encouragement! We all want to see you winning
 
Is this a lay down proper body scan?
Or the type you stand and hold handles?

Generally under 12% body fat has visible abs/muscle, so I'd say your higher than it says to be honest.

And being larger beforehand, your skin has stretched, so it won't be tight and hug the mid section like alot of other peoples.

I'd start training abs/core x3 per week and keep lifting. Help fill out the space with muscle to fill in under the skin.

Thanks for the response, bro. Yes, it was a proper professional DEXA scan — the lie-down type, not the handheld one. I can actually feel with my hand that the skin layer on my belly is really thin, but there’s a distinct balloon-like bulge pushing out from underneath. It doesn’t feel like fat, and it doesn’t seem normal to me. I do have a bit of loose skin, yeah, but this inner "balloon" is what’s really causing the protrusion. Even if my skin tightened, the bulge would still be there. That’s what’s confusing me.

This could be bloat or it could also be fat around the organs causing this protrusion. The issue is you really don't have any muscles to protrude out in the chest and offset this. You need to work on core control/strength. Pull it in slightly at all times. Keep it under control.
To check if it's bloat drop carbs for a solid week, drink a ton of water, small meals and get a ton of fiber in. If it reduces significantly then it's bloat.
I have the same issue but I control my core and have a muscled frame so you can't really tell.

@DarkAthlete you do NOT need surgery man. come on. you need to get on a plan to work on core strength. you probably have poor posture and core. go do some pilates

bros you need to improve posture. everyday do stomach vacuums. you probably got very weak core as others aid

I kinda agree with this.

The weight loss journey to date is... not to fuck about... been outstanding.

But I did worry that its become an obsession. I also noticed that, just by way of example, your forearm is pretty much the same size as your wrist. Which will mean 7-inches tops. Probably less.

So it's time to hit the weights. And do planks. Bigger, more muscular (no need to be huge etc) and with a nice flattened low ab set

So as you can see, my belly fat is very low. Yet, my abdomen still sticks out — even first thing in the morning, on an empty stomach after using the toilet. There’s no bloating or digestive discomfort. When I touch my belly, it feels like there’s a thin layer of skin, and underneath that, a sort of “balloon-like” layer pushing outward. It doesn't feel like typical fat.

When I consciously pull my belly in, it flattens — but of course, that’s not natural and only happens when I hold it in.

Throughout the day, especially after meals, it expands even more, and by the evening, it often looks like I have a belly again, despite being lean. Strangers would probably assume I still need to diet.

What I’m Trying to Understand:

  1. Why doesn’t my stomach lie flat like others at a similar body fat percentage?
  2. Could this be due to weak core muscles? Will strengthening my core help flatten it?
  3. Is this a case of stretched fascia or a weakened transverse abdominis that can’t hold the organs in?
  4. Could it be diastasis recti? I’ve done the finger test in a crunch position and the abs feel fully connected.
  5. Do I possibly need surgery, like a tummy tuck (abdominoplasty), to repair or tighten the muscles?
  6. Is there anything else I might be overlooking?
Any advice, insights, or feedback from those who’ve experienced something similar — or professionals with knowledge in this area — would be deeply appreciated. Thank you so much for taking the time to read and help!
Truthfully I think it’s just your posture
 
You're not 8% bodyfat. I'm around 7% and we don't look similar.
That being said, diet and exercise like everyone else is saying.
See your doctor if you're really concerned though
so it means the Dexa scan is not right, I will do another dexa scan in a couple weeks in a different place. let us see the comparison.

in such case, do we go to GPs or cosmetic surgeons to diagnose?
